La consultazione psicoanalitica istituzionale: trovare, ri-trovare, trasformare

Questo lavoro nasce dall’esperienza di un gruppo di colleghi che ha effettuato per alcuni anni un lavoro di intervisione sui colloqui di consultazioni fatti per il Centro Veneto di psicoanalisi; Vengono messi in discussione dall’autore i fattori che hanno favorito o sfavorito il buon esito delle consultazioni, i modelli clinici che hanno preso forma. Se l’analista che conduce la consultazione, per una scelta fatta dal Servizio, non potrà prendere in cura il paziente ma lo dovrà inviare ad un collega, si pone la questione di come svolgere questa funzione di collegamento. Ad esempio quanto approfondire la conoscenza del paziente e quanto delegare al collega che riceverà l’invio? Si valuta inoltre quale possa essere, nel micro-processo consultivo, una corretta utilizzazione del metodo analitico. Quale ascolto e quale forma di attenzione? Come utilizzare il transfert e il controtransfert, inoltre come realizzare anche in questa fase un alleanza terapeutica.

Psychoanalytic consultancy in institutions: to find, to find again, to transform. Testing the method. This paper takes origin from the experience of a group of colleagues who carried on for several years a discussion group on the consultancy session carried out for the «Centro Veneto di Psicoanalisi». In this paper we question the factors that have favored or disadvantaged the good result of the consultancies and the clinical models which took shape. If the analyst who carries on the consultancy, due to an institution’s choice, will not carry on with a treatment and will have to send the patient to a colleague, it will be necessary to face the problem of how to carry out a linking function. For example, how much to dig in depth with the aim of knowing the patient, and how much to delegate to the colleague who will take the patient in treatment? It is also considered, inside the micro-process of consultancy, which use of psychoanalytic method could be most appropriate. Which way of listening, and which kind of attention? How to use transference and countertransference? How to build up a therapeutic alliance since this very beginning?

La consultation psychanalytique institutionnelle: trouver, retrouver, transformer La méthode à l’épreuve. Ce travail découle de l’expérience d’un groupe de collègues qui ont effectué un travail d’intervision pendant quelques années sur les consultations tenues pour le Centre de Psychanalyse de Vénétie; Les facteurs qui ont favorisé ou échoué le succès des consultations, et les modèles cliniques qui ont pris forme, sont remis en question par l’auteur. Si l’analyste qui conduit la consultation, pour un choix fait par le Service, ne peut pas prendre en charge le patient mais doit l’envoyer à un collègue, se pose la question de savoir comment réaliser cette fonction de connexion. Par exemple, combien approfondir les connaissances du patient et combien déléguer au collègue qui va recevoir l’envoi? Il est également évalué quelle est l’utilisation correcte de la méthode analytique dans le micro-processus de consultation. Quelle écoute et quel genre d’attention? Comment utiliser le transfert et le contre-transfert ? Comment réaliser, dans cette phase aussi, une alliance thérapeutique.

La consulta psicoanalítica institucional: encontrar, re-encontrar, transformar. El método a prueba. Este trabajo nace de la experiencia de un grupo de colegas que ha efectuado durante algunos años un trabajo de intervención sobre las entrevistas de consulta realizadas por el Centro Veneto de psicoanálisis. El autor cuestiona los factores que han favorecido o desfavorecido el resultado positivo de las consultas, los modelos clínicos que han tomado forma. Si el analista que conduce la consulta, por una elección del Servicio, no podrá encargarse del paciente, pero lo tendrá que enviar a un colega, hay que preguntarse sobre la modalidad de desarrollo de esta función de contacto. Por ejemplo, cuánto profundizar el conocimiento del paciente y cuánto delegar al colega que lo recibirá después? Además, se evalúa cuál puede ser, en el micro-proceso consultivo, un uso correcto del método analítico. Cuál escucha y cuál forma de atención? Cómo utilizar el transfert y el controtransfert, y cómo realizar en esta fase una aleanza terapéutica.

Die institutionelle psychoanalytische Beratung: finden, wiederfinden, transformieren. Die Methode auf die Probe gestellt. Diese Arbeit entspringt der Erfahrung einer Gruppe von Kollegen, die einige Jahre lang kollegiale Beratungen zu den Beratungsgesprächen im Zentrum für Psychoanalyse in Venetien durchgeführt haben. Der Verfasser stellt die Faktoren zur Debatte, die das Gelingen der Beratung begünstigten oder erschwerten, die klinischen Modelle, die sich herausgebildet haben. Wenn der Analytiker, der die Beratung durchführt, aufgrund einer Entscheidung des Dienstes den Patienten nicht in Behandlung nehmen kann, sondern ihn an einen Kollegen verweisen muss, stellt sich die Frage, wie diese verbindende Funktion realisiert werden soll. Wie sehr etwa die Bekanntschaft mit dem Patienten vertiefen, wie viel an den Kollegen delegieren, an den der Patient verwiesen werden wird? Darüber hinaus wird ausgewertet, was im beratenden Mikroprozess eine korrekte Anwendung der analytischen Methode sein kann. Welches Zuhören und welche Form der Aufmerksamkeit? Wie sollen Übertragung und Gegenübertragung genutzt und wie soll auch in dieser Phase eine therapeutische Allianz realisiert werden?
