Psicoanalisi e urgenze psichiatriche

Si propone l’assunto che il metodo psicoanalitico e le conoscenze teoriche e tecniche che ha originato forniscano un aiuto peculiare nel trattamento della seria patologia mentale anche e soprattutto nell’urgenza psichiatrica. L’ascolto psicoanalitico, in particolare, consente di evidenziare aspetti della comunicazione verbale e non verbale, mettendo in luce le angosce profonde sia del paziente che dei curanti. Facilita così la ricostruzione immaginativa della identità del paziente: primo passo verso l’elaborazione di interventi terapeutici adeguati. Le vicende di un paziente ricoverato in un O.P.G. (Ospedale Psichiatrico Giudiziario) discusse con il personale di degenza-custodia in un gruppo psicoanalitico di riflessione clinica in tre differenti occasioni esemplificano tale tesi.

Psychoanalysis and Psychiatric Emergencies. This paper suggests that the psychoanalytic method and the theoretical and technical knowledge from which it originated provide a particular type of assistance in the treatment of serious mental pathology, even and especially in psychiatric emergencies. Psychoanalytic listening, in particular, allows aspects of verbal and nonverbal communication to be highlighted through a focus on the deep anxieties of both patient and caregivers. In this way, it facilitates an imaginative reconstruction of the patient’s identity, which is the first step toward the development of appropriate therapeutic interventions. Events during an inpatient’s stay at a state psychiatric hospital, discussed with members of the caregiving staff in a clinically oriented psychoanalytic group on three occasions, illustrate these topics.

Psychanalyse et psychiatriques d’urgence. On propose la thèse que la méthode psychanalytique et les connaissances théoriques et techniques qu’elle a générées fournissent une aide unique dans le traitement de la pathologie mentale grave, aussi et surtout dans l’urgence psychiatrique. L’écoute psychanalytique, en particulier, permet de mettre en évidence les aspects de la communication verbale et non verbale, en mettant en évidence les profondes inquiétudes du patient et de l’analyste. Ainsi, il facilite la reconstruction imaginative de l’identité du patient: première étape vers le développement d’interventions thérapeutiques appropriées. L’histoire d’un patient hospitalisé dans un O.P.G. (Hôpital psychiatrique judiciaire), discutée avec le personnel de l’hôpital dans un groupe psychanalytique de réflexion clinique dans trois occasions différentes, illustrent ce point de vue.

Psicoanálisis y urgencias psiquiátricas. Se propone la tesis de que el método psicoanalítico y los conocimientos teóricos, así como las técnicas que ha originado, ofrecen una ayuda peculiar en el tratamiento de la seria patología mental también y sobre todo en la urgencia pisquiátrica. La escucha psicoanalítica, en especial, permite subrayar aspectos de la comunicación verbal y no verbal, illuminando las angustias profundas tanto del paciente como de quien lo cura. Así, facilita la reconstrucción imaginativa de la identidad del paciente: primer paso hacia la elaboración de intervenciones terapéuticas adecuadas. Las vicisitudes de un paciente ingresado en un H.P.J. (Hospital Psiquiátrico Judicial) examinadas en tres diferentes ocasiones con el personal de hospitalización-tutela en un grupo psicoanalítico de reflexión clínica ejemplifican dicha tesis.

Psychoanalyse und psychiatrische Notfälle. Es wird vorgeschlagen, dass die psychoanalytische Methode mit ihren theoretischen und technischen Erkenntnissen eine spezifische Hilfe bei der Behandlung von schweren psychischen Störungen und insbesondere bei psychiatrischen Notfällen darstellen kann. Das psychoanalytische Zuhören ermöglicht das Erkennen von tiefen Ängsten sowohl der Patienten, als auch der Behandler. Somit erleichtert es den Wiederaufbau der Identität des Patienten, der ein erster Schritt für die Planung adäquater therapeutischer Schritte darstellt. Die Erfahrungen mit einem Patienten eines psychiatrisch-forensischen Krankenhauses, die von Seiten des Stationsteams im Rahmen einer psychoanalytischen Gruppe in drei Sitzungen reflektiert werden, veranschaulichen die dargestellte These.
