Le fobie e la tentacolare isteria

L’avvento delle tesi psicoanalitiche comporta una progressiva generalizzazione dei quadri delle fobie verso le isterie. Ciò accade attraverso il registro della sessualità, della neurastenia fino alla nevrosi di angoscia. Dal caso di Hans, le fobie sono finalmente assunte nel campo delle isterie e l’angoscia considerata sintomo da rimozione come nell’isteria. In seguito Freud, attraverso il ripensamento del concetto di angoscia, suggerisce una specificità delle fobie rispetto all’isteria riconoscendone un’organizzazione più antica, strutturata secondo «rimozioni originarie» precedenti la strutturazione del super-Io. Da alcuni anni la teorizzazione psicoanalitica riprende l’attenzione per la differenziazione delle fobie dall’isteria ponendola in una fase evolutiva presimbolica. Ciò allontana sempre più le fobie dal campo nevrotico avvicinandole ai quadri delle somatizzazioni, attacchi di panico, disturbi egosintonici, fino alle psicosi.

Phobias and Diffuse Hysteria. The advent of psychoanalytic theories has entailed a progressive generalization of the view of phobias in the direction of hysteria. That happens through the register of sexuality, of neurasthenia to the point of anxiety neurosis. Since the case of Little Hans, the phobias have ultimately been subsumed in the field of hysteria and of anxiety considered to be a symptom of repression, as in hysteria. After Freud, through rethinking the concept of anxiety, a particular definition of the phobias with respect to hysteria has been suggested, recognizing an earlier organization that is structured according to «originary repressions», preceding the structuralization of the superego. For some years, psychoanalytic theorization has again focused on the differentiation of the phobias from hysteria, placing it in a presymbolic developmental phase. This further distances phobias from the neurotic field, approaching a somatic framework, panic attacks, and ego-syntonic disturbances to the point of psychosis.

Les phobies et l’hystérie tentaculaire. L’avènement des théories psychanalytiques implique une généralisation progressive des cadres des phobies vers les hystéries. Cela se produit à travers le registre de la sexualité, de la neurasthénie jusqu’à la névrose d’angoisse. Dès le cas de Hans, les phobies sont finalement prises dans le domaine de l’hystérie et l’angoisse est considérée en tant que symptôme de refoulement, comme dans l’hystérie. Plus tard Freud, en repensant le concept d’angoisse, suggère une spécificité des phobies par rapport à l’hystérie, en en reconnaissant une organisation plus ancienne, structurée selon de «refoulements originaires» précédentes la structuration du Surmoi. Depuis plusieurs années, la théorisation psychanalytique reprend l’attention pour la différenciation des phobies de hystérie, en la plaçant dans une phase présymbolique de l’évolution. Cela eloigne les phobies de plus en plus du champ névrotique, en les rapprochant aux somatisations, aux attaques de panique, aux cadres egosyntoniques, jusqu’à aux psychoses.

Las fobias y la histeria tentacular. El adviento de las tesis psicoanalíticas implica una progresiva generalización de los cuadros de las fobias hacia las histerias. Esto sucede a través del registro de la sexualidad, desde la neurastenia hasta la neurosis de angustia. A paritr del caso de Hans, las fobias han sido finalmente asumidas dentro del campo de las histerias, y la angustia ha sido considerada síntoma de represión como en la histeria. Más tarde Freud, a través del cuestionamiento del concepto de angustia, sugiere una especificidad de las fobias respecto de la histeria, reconociendo en las primeras una organización más antigua, estructurada según represiones originarias precedentes a la estructuración del súper-Yo. Desde hace algunos años la teorización psicoanalítica ha vuelto a dirigir la atención en la diferenciación entre las fobias y la histeria, colocando las primeras en una fase evolutiva presimbólica. Esto aleja cada vez más las fobias del campo neurótico acercándolas a los cuadros de somatizaciones, ataques de pánico, molestias/señales egosintónicas, hasta las psicosis.

Die Phobien und die «Tentakelartige» Hysterie. Die Einführung der psychoanalytischen Thesen bringt eine progressive Generalisierung der Bilder der Phobien in Richtung der Hysterien mit sich. Und zwar geschieht dies quer durch das Register der Sexualität und der Neurasthenie bis hin zu den Angst-Neurosen. Seit dem klinischen Fall vom «kleinen Hans» werden die Phobien schließlich in das Gebiet der Hysterie aufgenommen und als Symptom der Verdrängung wie bei der Hysterie angesehen. In der Folge kommt Freud, durch seine neue Auslegung des Konzepts der Angst, auf eine Spezifität der Phobien in Bezug auf die Hysterie, und dabei würdigt er eine ältere Organisationsform, gemäß der «ursprünglichen Verdrängungen», welche der Strukturierung des Überichs vorausgehen. Seit einigen Jahren wirft die psychoanalytische Theoretisierung wieder die Aufmerksamkeit auf die Differenzierung der Phobien von der Hysterie und setzt diese dabei in eine evolutive präsymbolische Phase. Dadurch entfernen sich die Phobien immer mehr vom Feld der Neurosen und nähern sich an die Bilder der Somatisierungen, Panikattacken und egosyntonischen Störungen bis schließlich zu den Psychosen an.
